In an effort to raise awareness for molecular imaging and its clinical
utility, the AMI is sponsoring a new educational web-based lecture
series on Positron Emission Tomography (PET) entitled "Molecular
Imaging for Impact". These webcasts are designed for imagers as well
as for referring physicians. Each webcast will cover a unique topic -
everything from current trends in lung nodule staging to new
indications for oncologic imaging and emerging practices in the
evaluation of coronary artery disease. These programs will begin this
month and will continue through 2009.

Be sure to inform your clinician colleagues about this educational
series and book your calendars for the inaugural live webcast on August
11th. The first lecture is entitled "PET/CT Imaging in Esophageal
Cancer" on August 11th at 1:00 pm Eastern Time. Our speaker is Steve
Larson, MD, of Memorial Sloan-Kettering Cancer Center.

Ya está colgado en el blog de noticias PET-TAC el pdf de la resolución de la Comunidad de Madrid por la cual se adjudican 4 nuevas cámaras PET-TAC en la red hospitalaria pública madrileña. La relación definitiva de los 6 hospitales públicos en Madrid con equipos PET-TAC queda así:

Esperamos que el resto de Comunidades Autónomas sigan el ejemplo de Madrid y la técnica sea incorporada de forma definitiva y sin limitaciones burocráticas en el ambiente hospitalario tanto público como privado

El siguiente reto es conseguir que otros radiofármacos PET distintos de la FDG puedan ser utilizados también de forma rutinaria y sin los problemas que actualmente existen. Confiemos en ello

The Academy of Molecular Imaging has a new mail forum that has been up and running for about 45 days. Currently over 300 people have moved to the new mail forum, but some people are still using the old forum. National Oncologic PET Registry (NOPR) Data Confirm FDG-PET Has Major Impact on Management of Cancer

Los Angeles, Calif., Mar. 25, 2008 — Clinicians changed the intended care of more than one in three cancer patients as the result of FDG-PET scan findings, according to a study of data from the National Oncologic PET Registry, published online March 24 in the Journal of Clinical Oncology (JCO).

The study analyzed data regarding nearly 23,000 patients contributed to the NOPR by more than 1200 facilities nationwide providing positron emission tomography (PET) scans.

”The NOPR working group sought to measure the impact of PET findings on patient management in a manner minimally intrusive to care providers. This was critical for successfully collecting the large amount of data required for a robust analysis,” said Bruce Hillner, M.D., lead author for the study and professor and eminent university scholar in the Department of Internal Medicine at Virginia Commonwealth University.
Sponsored by the Academy of Molecular Imaging (AMI) and managed by the American College of Radiology (ACR) and the ACR Imaging Network (ACRIN), the NOPR was designed to collect questionnaire data from referring physicians on intended patient management before and after a FDG-PET scan.
The NOPR participating PET facility collects from referring physicians both a pre-PET questionnaire (documenting study indication, cancer type and anticipated stage, and planned management if PET were not available) and one of several post-PET questionnaires that assess the referring physician’s planned management in light of the FDG-PET findings. Analysis of data collected found that FDG-PET is associated with a 36.5% change in the decision of whether or how to treat a patient’s cancer. NOPR working group co-chair R. Edward Coleman, MD, professor of radiology and chief of the Division of Nuclear Medicine at Duke University School of Medicine and an AMI founding member, comments, “We were especially surprised by the impact of the PET findings on patients who were originally planned to have a biopsy. The procedure was avoided in approximately three-quarters of these patients.” The NOPR was launched in May 2006 in response to the Center for Medicare and Medicaid Services’ (CMS) novel “Coverage with Evidence Development” policy to collect data through a clinical registry to inform the center’s FDG-PET coverage determination decisions for currently non-covered cancer indications. Cancer types Medicare currently covers for reimbursement only through the NOPR include those of the ovary, uterus, prostate, pancreas, stomach, kidney and bladder. Oncologist and NOPR working group co-chair, Anthony Shields, MD, professor of medicine and oncology at the Karmanos Cancer Institute at Wayne State University and chair of ACRIN’s Oncology Committee says of the research results, “These results confirm what we suspected from increasing experience with PET. However, we lacked the significant data required to prove the benefit of PET for many uncovered indications. It’s very encouraging that oncologists and other clinicians may have access to the valuable information PET affords for ensuring the best patient care.” NOPR has formally asked CMS to reconsider the current National Coverage decision on FDG-PET and to end the data collection requirements for diagnosis, staging and restaging. Medicare will review the published data and determine the next steps related to reimbursement for PET scans now only covered through the NOPR. Barry Siegel, MD, FACR, professor of radiology and chief of the Division of Nuclear Medicine at the Mallinckrodt Institute of Radiology at Washington University and chair of ACRIN’s PET Imaging Core Laboratory, also serves as an NOPR working group co-chair. "Based on these data, Medicare should strongly consider opening up the coverage to include diagnosis, staging and restaging for all cancers, states Dr Siegel.” FDG-PET, also called PET imaging or PET scan, is a test that images the function of cells to show differences between healthy tissue and diseased tissue. It uses a small amount of a radioactive chemical which is combined with sugar. This combination is called FDG, so the test is sometimes called an FDG-PET scan. It is used to evaluate various neurological and cardiac disorders, as well as for diagnosing, staging, and monitoring the treatment of many different cancers. The ACR and ACRIN worked to develop the NOPR in collaboration with registry sponsor, the Academy for Molecular Imaging since CMS announced its intent to support a PET registry in January 2005. The American Society of Clinical Oncology and the Society for Nuclear Medicine also have played key roles in guiding the project’s development.

V Reunion del Grupo PET de la SEMN

La Sociedad Española de Medicina Nuclear a través de su grupo PET, viene desarrollando en los últimos años un programa de actividades científico-técnicas encaminadas a promover el uso adecuado y racional de la tecnología PET. En estas actividades se enmarca esta V Reunión del Grupo PET, que con unos criterios organizativos muy clínicos, pretende revisar las aplicaciones de la PET-TAC en el cáncer de Pulmón. Esta patología constituye sin duda uno de los desafíos mas importantes planteados a la moderna Oncología, y solamente puede ser abordado desde un enfoque multidisciplinar.
Con la ilusión de que los resultados de esta reunión nos ayudarán a utilizar mejor la PET-TAC en el manejo de los pacientes con Ca de Pulmón, os invito a participar en la misma y os doy la bienvenida anticipada a Sevilla .

El pet (tomografía por emisión de positrones) es una técnica dediagnóstico que permite obtener imágenes que muestran el metabolismo yel funcionamiento de tejidos y órganos. Se utiliza con fines clínicosen oncología, pero también en neurología y cardiología. Al paciente sele inyecta un fármaco radiactivo (un derivado fluorado de la glucosa,FDG), que se distribuye por todas las células del organismo, pero seconcentra donde el metabolismo celular está aumentado. El radiofármacoemite positrones, idénticos a los electrones, pero con carga positiva,que se aniquilan con los electrones del medio produciendo radiacióngamma, que se detecta desde el exterior y permite localizar ycuantificar la zona.Hoy se tiende a usar aparatos híbridos pet-tac. El punto débil del petes la información anatómica. Las lesiones se aprecian como áreasaisladas, sin poder precisar la localización exacta del tumor, ni suextensión, ni sus límites morfológicos. Sin embargo, el tac(tomografía axial computarizada) aporta una extraordinaria resoluciónmorfológica, lo que permite definir correctamente dónde se estágestando el tumor y delimitar su tamaño. El ciclotrón (acelerador departículas) se usa para producir flúor radiactivo, que mantiene suspropiedades radiactivas durante un período de tiempo relativamentebreve, por eso debe estar próximo al sistema pet-tac. En consecuencia,es necesario poner en marcha el tándem ciclotrón-pet-tac para obtenerel máximo rendimiento de la instalación.

Por ser de interés general informamos que se ha publicado el Real Decreto 1345/2007, de 11 de octubre, por el que se regula el procedimiento de autorización, registro y condiciones de dispensación de los medicamentos de uso humano fabricados industrialmente (BOE de 7 de noviembre de 2007).
A este respecto ya comentamos anteriormente sobre tres aspectos que afectan a los radiofármacos como a cualquier medicamento (<http://www.agemed.es/> <http://correoweb.msc.es/exchweb/bin/redir.asp?URL=http://www.agemed.es/%253E> en "Actividad" "Actividades y congresos" "Artículos científicos"):

Los radiofármacos están considerados como medicamentos especiales, y obligados a contar con autorización previa de la AEMPS para su uso humano.
*
La evaluación previa a la autorización de cualquier medicamento es indispensable para asegurar que su uso humano se realizará en condiciones que cumplen las garantías de calidad, seguridad, eficacia, identificación e información que exige la normativa española y comunitaria."
*
Los ensayos clínicos con medicamentos en investigación, incluyendo los radiofármacos, estarán sometidos al régimen de autorización previa por la AEMPS para garantizar dicha investigación, conforme al procedimiento reglamentariamente establecido.
En el Real Decreto 1345/2007 se recogen los medicamentos especiales que contaban con su propia legislación y que quedan ahora integrados en esta misma norma. Se deroga el Real Decreto 479/1993, de 2 de abril, por el que se regulaban los medicamentos radiofármacos de uso humano.

OBJECTIVE: The aims of the study were to (1) evaluate the range of physiological FDG uptake in normal pharyngeal palatine tonsil, and (2) investigate the possibility of establishing a cut-off threshold to distinguish between normal pharyngeal palatine tonsil FDG uptake from occult pharyngeal palatine tonsil primary cancer. METHODS: FDG PET CT of 43 consecutive patients with a low risk of head and neck cancer were reviewed by two observers. Axial PET CT was used to identify foci of FDG uptake related to the pharyngeal palatine tonsil. The highest standardized uptake value, SUVmax, of the left and right pharyngeal palatine tonsil was calculated. Similar analysis was performed on 10 consecutive patents with histologically proven occult pharyngeal palatine tonsil primary cancer. RESULTS: The mean SUVmax of the 43 right pharyngeal palatine tonsils was 4.82 (range, 1.16-12.74) and 4.68 (range, 0.88-13.65) for the 43 left pharyngeal palatine tonsils with no statistical difference observed (P=0.4). Normal pharyngeal palatine tonsil uptake was generally symmetrical and there was a positive correlation between SUVmax from the left and right sides which was statistically significant (r=0.9, P<0.0001).>

Madrid invertirá 8,1 millones de euros en 4 nuevos PET

El Consejo de Gobierno de la Comunidad de Madrid acordó ayer destinar 8.160.000 euros a la adquisición de cuatro nuevos equipos de tomografía de emisión de positrones (PET), lo que convierte a Madrid "en la región con más equipos de esta tecnología de toda España", según Ignacio González, portavoz del Gobierno regional.

Our radiation therapists are interested in having our PET/CT dept acquire limited field of view pet/ct scans on a flat bed in order to fuse that data with their CT simulator to do RT planning. I am of the understanding that there is a code for limited PET/CT but will the insurers pay if a routine PET/CT was done a few weeks prior to the limited one for RT planning? How do other PET/CT departments handle this issue?

Dear Members of the EANM,
Dear Colleagues,
Following the publication on the EANM website of the "White paper of the EANM and the European Society of Radiology (ESR) on multimodality imaging", the EANM Executive Committee has received quite a few emails and letters, mainly from French and Belgian colleagues, protesting against the white paper and asking for its publication in the August issue of the EJNMMI to be stopped.
During a recent EANM Executive Committee meeting, the issue was again discussed, and the Executive Committee unanimously stands behind the "EANM/ESR white paper on multimodality imaging", which was written by the leaderships of both EANM and ESR in order to give a vision for cooperation, which will undoubtedly be necessary between the two associations to encompass imaging techniques of the future.
So far, the EANM Executive Committee has received very favourable comment on the white paper from the Dutch and the Italian Society, as well as the industry association AIPES and the leadership of the SNM.
Therefore, the EANM Executive Committee unanimously decided to go ahead with the publication of the white paper in the August issue of the EJNMMI.
In order to provide a forum to discuss issues of future interest, the EANM Executive Committee has programmed a round table entitled "Multimodality imaging and the future of nuclear medicine in Europe" on Monday, October 15, 2007, 11:30 - 13:00, during the EANM Congress in Copenhagen.
We would like to invite all interested EANM members to attend this session and look forward to an enlightening discussion in Copenhagen.

PET/CT predicts chemo response in lung cancer patients FDG-PET/CT can accurately tell which lung cancer patients will have a successful response to chemotherapy, and therefore a longer survival, and which will not, according to a study in the May issue of the Journal of Nuclear Medicine. Findings may have important implications for patient management.
Chemotherapy, alone or combined with radiotherapy, remains the standard of care for patients with inoperable non-small cell lung cancer. Chemo does not work in every case, however, subjecting some patients to unnecessary toxicity and severe side effects. The ability to provide an early indication of therapeutic response could improve patient care by identifying those patients who are not benefiting from their current treatment, said principal investigator Dr. Claude Nahmias, a professor of radiology and medicine at the University of Tennessee Medical Center in Knoxville.
"Although we studied a relatively small number of patients, it is clear that a repeat PET study with FDG at the end of the first cycle of chemotherapy would allow the identification of those patients for whom the therapy was futile," he said.
Nahmias and colleagues prospectively assessed 16 patients with non-small cell lung cancer as they received two courses of treatment with docetaxel and carboplatin. Patients underwent weekly FDG-PET/CT scanning over seven weeks, starting right before the first chemo dose.
The investigators found that PET/CT scans performed at one and at three weeks accurately indicated which patients would successfully respond to treatment.
Fifteen patients completed the study. Eight did not respond and died within 35 weeks after the completion of the study. Seven patients were classified as responders. Five survived, while one died at 25 weeks and another at 76 weeks. A 0.5 decrease in FDG standardized uptake value at one and three weeks scanning correlated with survival longer than six months.
By the end of the first cycle of chemotherapy, FDG-PET/CT would allow identification of patients who will not benefit from the treatment.
These patients could benefit by either stopping chemo or by entering a different, more adequate therapeutic approach, Nahmias said.
The small patient sample calls for caution in interpreting results.
This study needs validation by a larger trial, but it demonstrates that patients who respond to chemotherapy and those who do not can be identified early in the course of their treatment, Nahmias said.
"I am forever grateful to all the patients who came back week after week to undergo our PET scans," he said.

Patients with nonsmall-cell lung cancer, a dedicated FDG-PET with the maxSUV of the primary lung tumor and FDG-avid mediastinal (N2) nodes reported (before therapy), and who underwent lymph node removal were eligible. RESULTS: There were 239 patients with 335 FDG-PET-positive N2 nodes at 14 different PET centers. The median ratio of the maxSUV of the lymph node to the maxSUV of the primary tumor of the pathologically proven malignant nodes was 0.58 (range, 0.32 to 1.61). Benign nodes had a median ratio of 0.40 (range, 0.21 to 1.10, p = 0.02). The median value was similar for all centers except one. Receiver operating characteristics analysis determined the optimal value of the ratio that maximized sensitivity to be 0.56 or greater (+LR 6.6, sensitivity 94%, specificity 72%). CONCLUSIONS: The ratio of the maxSUV of the mediastinal (N2) lymph node to the maxSUV of the primary tumor in patients with nonsmall-cell lung cancer predicts mediastinal nodal pathology across different PET centers. When the ratio is 0.56 or greater, there is a 94% chance that the node is malignant. The ratio may take into account the different techniques used at different centers

La PET-TC debe ser el método diagnóstico de elección en la estadificación inicial del carcinoma de pulmón no microcítico, una de las 23 indicaciones incluidas desde 2006 en la cartera de servicios comunes del Sistema Nacional de Salud.Uno de sus papeles más relevantes lo juega en el diagnóstico inicial de metástasis a distancia, ya que detecta entre un 10 y un 15 por ciento de lesiones secundarias no conocidas por otros métodos de imagen, una variación en el diagnóstico que obliga a replantear el tratamiento.Así, en los casos de pacientes a los que se había establecido una indicación de cirugía y en los que con la aplicación del PET-TC se detectan lesiones no sospechadas será necesario reorientar el tratamiento, eliminando la opción quirúrgica y estableciendo un tratamiento con quimioterapia, "con lo que se evitan los costes derivados de una cirugía que no iba a dar los resultados esperados, y también puede suceder que pacientes a quienes se había establecido como pauta terapéutica la quimioterapia sean finalmente subsidiarios de recibir cirugía", según ha señalado Antonio Maldonado, coordinador médico de la División PET del Grupo Recoletas. El experto ha participado en una ponencia sobre el valor diagnóstico de la asociación PET-TC en cáncer de pulmón, en el XII Congreso de la Sociedad Asturiana de Patología Respiratoria, celebrado en Oviedo.Utilidad a otros nivelesDe especial interés destaca el papel de la PET-TC en la estadificación mediastínica, permitiendo una correcta clasificación de las regiones ganglionares afectadas, evitando en algunos casos otros métodos diagnósticos invasivos. "Gracias a la unión de la imagen metabólica PET y la anatómica TC, es posible obtener en algunos casos una mejor delimitación del tumor primario, con una mejora en el rendimiento de la estadificación TNM del 20 por ciento".La PET-TC ha demostrado también su utilidad en la determinación de los nódulos pulmonares, para establecer si son o no malignos, "ya que uniendo ambas técnicas obtenemos de forma simultánea la información morfológica y metabólica del nódulo".Por otra parte, si se quiere determinar la actividad tumoral para valorar la evolución del tratamiento, la PET-TC, con un rendimiento de entre el 87 y el 90 por ciento, es también la prueba de elección.Por último, su uso en la planificación de los tratamientos radioterápicos, como la tomoterapia, para delimitar de forma más exacta el volumen tumoral a tratar, permite "dar una mayor dosis de tratamiento, con menos efectos secundarios y un mayor control local de la enfermedad".Ayuda con la radioterapiaLa introducción de la PET-TC en la planificación de la radioterapia supone un cambio de hasta el 53 por ciento de los pacientes comparado con la planificación actual con TC, según refiere la literatura."En algunos centros americanos, como es el caso del MD Anderson en Houston (Texas), de las ocho cámaras PET-TC que poseen, una está dedicada únicamente para la planificación de radioterapia".Los expertos defienden la necesidad de un PET-TC por cada dos millones de habitantes, ratio que aún no se ha alcanzado en España, donde actualmente están operativos 49 dispositivos, doce de ellos en la sanidad pública, "aunque esta cifra aumentará rápidamente porque tanto la sanidad pública como la privada están apostando por esta tecnología, dado su mayor rendimiento en el campo de la oncología frente al resto de métodos diagnósticos habituales", ha concluido Maldonado.

Los oncólogos no somos optimistas patológicos, pero disfrutamos mucho de las buenas noticias que afectan a nuestro quehacer.
En septiembre de 2006, el Ministerio de Sanidad y Consumo incluyó en la cartera de servicios de nuestro Sistema Nacional de Salud las indicaciones de uso que se contemplan en las especificaciones de la ficha técnica del radiofármaco F-fluorodesoxiglucosa ( F-FDG) elaborada por la Comisión Europea en 2005 después de analizar la evidencia científica bibliográfica disponible en los últimos 10 años.
Se trata de 26 situaciones clínicas en las que la tomografía por emisión de positrones (PET) contribuye significativamente a orientar la práctica asistencial y que se pueden agrupar en 4 categorías en relación al proceso asistencial oncológico:
1. Diagnóstico: caracterización del nódulo pulmonar solitario, detección de tumor de origen desconocido y caracterización de masa pancreática.
2. Estadificación: cáncer de cabeza y cuello, cáncer de pulmón, cáncer de mama localmente avanzado, cáncer de esófago, cáncer de páncreas, cáncer colorrectal, linfoma maligno, melanoma con Breslow mayor a 1,5 cm o metástasis ganglionares de inicio.
3. Monitorización de la respuesta al tratamiento: linfoma maligno y cáncer de cabeza y cuello.
4. Sospecha fundamentada de recidiva tumoral en el seguimiento: glioma de alto grado, cáncer de cabeza y cuello, cáncer de tiroides no medular con incremento de niveles séricos de tiroglobulina y estudio con yodo radiactivo corporal negativo, cáncer de pulmón, cáncer de mama, cáncer de páncreas, cáncer colorrectal, cáncer de ovario, linfoma maligno y melanoma.
Ésta es una noticia extraordinaria que merece ser ampliamente divulgada y debemos reflexionar sobre su impacto potencial en el presente y el futuro de la medicina oncológica. El cáncer, como realidad sociosanitaria, es un reto abrumador para los sistemas de cobertura y cuidados de la salud. La financiación de su atención asistencial y la adaptación al dinamismo del conocimiento/necesidad que genera es una pesadilla interminable para los gestores. La medicina del cáncer se concreta en un proceso asistencial con una secuencia de etapas clínicas que aseguran el estudio y el tratamiento de los pacientes con rigor y sentido de la anticipación: sospecha, diagnóstico, estadificación, tratamiento, seguimiento, rehabilitación/reinserción o en el caso de ausencia de control de cáncer inician un bucle asistencial de re-estadificación, re-tratamiento, cuidados continuos y/o finales.
Existe un convencimiento clínico respecto al cáncer humano que está transformando su asistencia médica: el cáncer es heterogéneo en sus niveles biológicos macro-micro-nanos y se desarrolla en individuos biológicamente heterogéneos (pluripatología, comorbilidad y exposición a factores de riesgo). La medicina oncológica evoluciona hacia la individualización. El uso sistemático de la tomografía por emisión de positrones y la tomografía axial computarizada (PET-TAC) en la asistencia oncológica añade elementos de calidad diferencial en el estudio individualizado de los pacientes. Aporta información, no disponible con ningún otro sistema alternativo, respecto a morfo-metabolismo del cáncer, topografía de su extensión en órganos y sistemas y orienta en la discriminación de imágenes de difícil interpretación exclusiva radiológica.
La validez de estas contribuciones puede extenderse a las etapas terapéuticas, de re-estadificación y valoración de la respuesta, seguimiento y controles periódicos. Es importante que los especialistas en Medicina Nuclear entiendan el trasfondo de individualización que gobierna la práctica oncológica para añadir la máxima información diferencial que el PET-TAC obtiene en "ese" paciente en particular. Ni en el cáncer humano en estadios precoces (altamente controlables) ni en los estadios avanzados, el oncólogo clínico pierde su interés por conocer y registrar el máximo de información individualizada que pueda afectar a sus decisiones asistenciales con "ese" enfermo en particular.
Los fundamentos de conocimiento que soportan la práctica asistencial son los elementos nucleares de la docencia en medicina especializada. La práctica clínica se dinamiza con el acceso a conocimientos/tecnología que complementa lo ya conocido. El PET-TAC en Oncología, en su desarrollo actual y de futuro, requiere, al menos, tres niveles urgentes de esfuerzo docente: evaluación e interpretación de resultados asistenciales individualizados entre especialistas en Oncología y Medicina Nuclear; enseñanza prospectiva de información actualizada entre médicos especialistas en formación; expansión de los contenidos y créditos docentes en los estudios de pregrado en Medicina. Enseñar las calidades del PET-TAC consolidará su implantación y desarrollo en la medicina del cáncer.
El PET-TAC será una oportunidad oncológica excitante de investigación biomédica en la próxima década. Correlacionar las características de la imagen molecular con la expresión de perfiles moleculares en cáncer humano, contribuirá a entender la heterogeneidad tumoral y su impacto en patrones evolutivos y de pronóstico. Estudiar los cambios producidos en bioimagen y expresión biomolecular por acción de la quimioterapia y/o radioterapia permitirá investigar los fenómenos de sensibilidad, reparación y resistencia asociados a estas terapéuticas. El PET-TAC estará presente en el estudio de la recidiva tumoral y nuevamente podrá correlacionarse la información PET con la expresión molecular del tejido tumoral recidivado (biorresistencia con progresión).
Como investigador clínico me interesa especialmente el desarrollo de modelos experimentales basados en la práctica clínica que permitan identificar estrategias innovadoras para mejorar la asistencia con pragmatismo: a los oncólogos y médicos nucleares de nuestra generación nos demandan progresar con los recursos disponibles en el entorno y en los pacientes concretos que se cruzan en nuestro camino profesional. Es difícil pensar en un desarrollo más práctico informativo que la generalización del uso del PET-TAC en oncología.
Deseo finalizar esta reflexión transmitiendo un convencimiento personal, que tiene un balance inestable entre especulación y certeza. El acceso facilitado de los oncólogos a la tecnología PET-TAC impulsa con realismo un modelo de acción médica de futuro que podría denominarse: "Oncología guiada por PET-TAC". Existen líneas de investigación clínica que exploran la cirugía oncológica guiada por radiofármacos que puede extenderse a proyectos prospectivos de evaluación de cirugía de cáncer planificada y ejecutada con estudios de bioimagen. Existe instrumentación comercialmente disponible que permite planificar la radioterapia integrando la imagen PET-TAC en el proceso de valoración y elaboración dosimétrica. Los análisis preliminares de la variabilidad de la práctica radioterápica al disponer de planificación dosimétrica guiada por imagen PET-TAC es tan acusada (15-63 % en el modelo de cáncer de pulmón) que convierte este desarrollo en imperativo para la radioterapia oncológica asistencial. Este elemento de optimización radioterápica justificaría, por sí mismo, la generalización de la implantación de tecnología PET-TAC en los hospitales con intensa actividad en Oncología radioterápica. La evidencia creciente de que la respuesta a fármacos antitumorales es detectable precozmente mediante variaciones del SUV (standard uptake value) al tratamiento quimioterápico puede definir un patrón evolutivo favorable (identificar respondedores) que permitirá también incluir el PET-TAC como guía de la práctica en Oncología médica.
Don Gregorio Marañón nos transmitió, en un prólogo perdido a un libro de un discípulo, que "lo difícil, lo importante, no es ejecutar técnicas, sino saber plantearse los problemas que haya detrás y juzgarlos con espíritu científico, que sólo se adquiere con la cultura". La cultura de nuestra época está dominada por la fusión del conocimiento. La bioingeniería imita esta tendencia. La tecnología actual es mestiza.
El PET-TAC es un desarrollo asistencial de la imagen médica que necesita especialistas cultos ("interesados por juzgar con espíritu científico los problemas que hay detrás de la técnica") para mejorar el cuidado de los enfermos oncológicos. Mi enhorabuena a la Medicina Nuclear y a la medicina oncológica por la accesibilidad a este excelente recurso médico trascendental.

To evaluate the additional value of contrast-enhanced multi-phase CT in comparison with low-dose non-contrast CT in combined PET/CT protocols for oncological imaging we retrospectively analysed 100 patients with different malignant tumours. All patients underwent a PET/CT consisting of a multi-phase CT protocol including a low-dose non-enhanced attenuation scan and an arterial and portal-venous contrast-enhanced scan followed by a whole-body PET. PET/CT studies were analysed by different categories to determine the added value of contrast-enhanced CT. The additional value was defined as new information provided by diagnostic CT and not available by the low-dose CT, resulting in change of PET/CT interpretation. The results were validated either by histopathology or by clinical-radiological follow-up >/=6 months. The clinical impact was evaluated with respect to changes in patient management. Diagnostic multi-phase CT was of additional value in 52 out of 100 patients with 85 suspected lesions. In 40 out of 100 patients no additional value could be detected. Eight patients were excluded due to inconclusive diagnosis in both methods including fusion. The analysis showed the greatest benefit of diagnostic CT in the categories localization of pathological FDG uptake and precise tumour delineation, changing PET/CT interpretation in 42% and 31% of patients, respectively. The benefit of diagnostic CT was influenced by the tumour type demonstrating the highest impact in gastrointestinal, lung and neuroendocrine tumours. Diagnostic CT changed clinical management in 21 patients (21%). Diagnostic multi-phase CT as part of the combined PET/CT protocol has the potential to provide considerable additional value in specific clinical conditions with resultant change of management in a substantial proportion of patients.

OBJECTIVE: Protocols for PET/CT are not yet standardized. In particular, image quality, utilization, and reporting of the findings of the CT component of PET/CT can vary widely, making it complicated for physicians to request the appropriate information. In an effort to address this problem, we describe a set of four PET/CT protocols that satisfy a broad range of clinical needs among oncology patients. Current technology allows acquisition of diagnostic-quality CT scans as part of PET/CT examinations, and referring physicians are given the option of requesting formal interpretation of the CT findings. In this case, the PET and CT images are interpreted by the corresponding specialists, and equivocal or discordant findings are adjudicated through joint review of the PET/CT images. CONCLUSIONS: The menu of PET/CT imaging protocols has gained wide acceptance by our referring physicians and have been used successfully in more than 6,000 PET/CT studies. Newer PET/CT protocols will be developed as technology advances. Continued collaboration among oncologists, CT specialists, and nuclear medicine specialists is essential for deriving the maximum clinical benefit from combined PET/CT. Standardization of imaging protocols will become increasingly important as multiple-institution trials are developed for evaluation of present and future applications of PET/CT.

Uso del sistema híbrido PET/TC en oncología: perspectiva radiológica

Objetivo. Describir nuestra experiencia con la técnica combinada tomografía por emisión de positrones-tomografía computarizada
(PET/TC) en el manejo de la patología oncológica. Material y métodos. El equipo PET/TC combina en un sistema híbrido un escáner helicoidal multicorte y un escáner PET. La técnica es multidisciplinar interviniendo un médico nuclear, un radiólogo, un físico y un radiofarmacéutico, con funciones diferenciadas. Las indicaciones de la PET se determinan bajo uso tutelado en el Sistema Nacional de Salud (SNS), coordinado por la Unidad de Evaluación de Tecnologías Sanitarias de la Agencia Laín Entralgo, bajo la supervisión y realización de un médico nuclear ayudado si se requiere para interpretar la TC por un radiólogo. Cuando se usa como estudio combinado, los estudios son supervisados por un médico nuclear y un radiólogo. Resultados. Entre septiembre de 2003 y agosto de 2005 hemos efectuado 2.459 estudios: 2.200 como PET, con TC de baja dosis (indicaciones clínicas bajo uso tutelado en el SNS) y 259 como estudios realizados en el marco de proyectos de investigación utilizándose, en estos
casos, como sistema híbrido en una única sesión. Los resultados globales muestran los siguientes porcentajes: cáncer de pulmón 14,7%,
nódulo pulmonar solitario 7%, neoplasias linfoides 23,5%, cáncer colorrectal 18,5%, cáncer gástrico 1,9%, tumores cerebrales 2,6%, tumores
de cabeza-cuello 5,5%, tiroides 5%, mama 4%, tumores de origen desconocido 4,3%, epilepsia 1,4% y otros 11,6%. Conclusión. En nuestra experiencia el sistema híbrido PET/CT aporta ventajas en comparación con la PET y/o TC aisladamente, pero son necesarios estudios de validación diagnóstica, que estamos realizando en indicaciones clínicas concretas.

PURPOSE: Standardized response criteria are needed to interpret and compare clinical trials and for approval of new therapeutic agents by regulatory agencies. METHODS: The International Working Group response criteria (Cheson et al, J Clin Oncol 17:1244, 1999) were widely adopted, but required reassessment because of identified limitations and the increased use of [(18)F]fluorodeoxyglucose-positron emission tomography (PET), immunohistochemistry (IHC), and flow cytometry. The International Harmonization Project was convened to provide updated recommendations. RESULTS: New guidelines are presented incorporating PET, IHC, and flow cytometry for definitions of response in non-Hodgkin's and Hodgkin's lymphoma. Standardized definitions of end points are provided. CONCLUSION: We hope that these guidelines will be adopted widely by study groups, pharmaceutical and biotechnology companies, and regulatory agencies to facilitate the development of new and more effective therapies to improve the outcome of patients with lymphoma

A 59-year-old woman, who had surgery for a suspected pancreatic tumor and was detected to have a gastrinoma, was referred for F-18 FDG PET for disease evaluation. Whole-body FDG PET showed diffuse intense FDG PET uptake (SUVmax 5.44) throughout the stomach. The patient was asked to drink a large amount of water and a repeat spot FDG PET (limited to the surface marking of the stomach) was acquired. The uptake was mildly reduced (SUVmax 4.75) but persisted in the distended stomach wall. An endoscopic biopsy from the antrum and fundus revealed follicular gastritis with erosion. The biopsy was negative for neoplasm. The patient was thought to have gastritis in a Zollinger-Ellison syndrome. Varying degrees of FDG uptake in the stomach have been described in certain benign conditions. Gastritis secondary to Zollinger-Ellison syndrome should be included in the differential diagnosis of diffuse intense gastric uptake, when such a history is present in the patient

Gastric carcinoma is the second leading cause of cancer-related death worldwide. Detection and surgical resection of gastric cancer in the early stage provides the only hope for improved survival in patients with gastric cancer. Positron emission tomography (PET) with F-18 2-deoxy-2-fluoro-D-glucose (FDG) has been shown to be essential in the evaluation of a variety of malignancies. However, conventional FDG PET has limited value for detecting a primary tumor of the stomach, mostly because of the relatively high levels of physiological uptake by the contracted stomach. We report 3 cases of primary gastric carcinomas detected successfully by FDG PET after the ingestion of food. The PET images of the stomach after ingesting food were compared with the routine fasting-state whole-body PET images for each patient. When the stomach was distended by food, the malignant lesions were more discernible. These cases indicate that gastric distension by ingesting food may be a simple method that can help to detect a primary gastric malignancy by FDG PET.

Goals and curriculum
EANM has established the educational facility in order to provide the highest standard of education. A renowned faculty was asked to prepare power point presentations and case studies to cover all courses offered by the EANM.
At each course instructors will cover the presentations in "classroom style" in the lecture room. There will also be ample time for participants to view case studies in 4 viewing rooms, equipped with state of the art multipurpose viewing stations as well as with workstations by all three major manufacturers of PET/CT equipment.

Purpose: To develop guidelines for performing and interpreting positron emission tomography (PET) imaging for treatment assessment in patients with lymphoma both in clinical practice and in clinical trials.
Methods: An International Harmonization Project (IHP) was convened to discuss standardization of clinical trial parameters in lymphoma. An imaging subcommittee developed consensus recommendations based on published PET literature and the collective expertise of its members in the use of PET in lymphoma. Only recommendations subsequently endorsed by all IHP subcommittees were adopted.
Recommendations: PET after completion of therapy should be performed at least 3 weeks, and preferably at 6 to 8 weeks, after chemotherapy or chemoimmunotherapy, and 8 to 12 weeks after radiation or chemoradiotherapy. Visual assessment alone is adequate for interpreting PET findings as positive or negative when assessing response after completion of therapy. Mediastinal blood pool activity is recommended as the reference background activity to define PET positivity for a residual mass 2 cm in greatest transverse diameter, regardless of its location. A smaller residual mass or a normal sized lymph node (ie, 1 x 1 cm in diameter) should be considered positive if its activity is above that of the surrounding background. Specific criteria for defining PET positivity in the liver, spleen, lung, and bone marrow are also proposed. Use of attenuation-corrected PET is strongly encouraged. Use of PET for treatment monitoring during a course of therapy should only be done in a clinical trial or as part of a prospective registry.

Si hay una rama de la medicina que está cambiando a un ritmo
vertiginoso y que ya está revolucionando el diagnóstico precoz de
muchas patologías y su tratamiento es la que comprende todas las
técnicas de imagen. Además de ser imprescindible para delimitar la
existencia de un tumor, ahora 'prometen' visualizar la posible
aparición del Alzheimer, antes incluso de que la efermedad dé
síntomas. Lo último en este tipo de pruebas es la fusión de dos
escáneres, el PET y el TAC, que ya están cambiando la forma de enfrentarse al cáncer

Mando referencia sobre articulo publicado en el NEJM sobre PET y Alzheimer, aparecido en DM de hoy. Para la Comunidad PET es siempre uan gran noticia que se publique en el NEJM. La alegría es mayor cuando el grupo que lo publica es en el que trabaja uno de los líderes mundiales del PET, el Prof. Jorge Barrio al cual le unen estrechos lazos con la SEMN y con algunos centros en nuestro pais. Científicos de la Universidad de California en Los Angeles (UCLA) han abierto por primera vez una ventana al cerebro del paciente con Alzheimer para distinguirlo del deterioro cognitivo leve.

Los investigadores han añadido a la tomografía por emisión de positrones
(PET) una pequeña molécula que se asocia con las proteínas anómalas implicadas en el mal de Alzheimer (beta amiloide y tau).
De esta forma, según publica hoy The New England Journal of Medicine, puede detectarse la enfermedad precozmente e incluso distinguirla del deterioro cognitivo leve.
Los artífices de esta nueva tecnología, coordinados por Gary Small Parlow-Solomon, profesor de Envejecimiento en el Instituto Semel de Neurociencia y Conducta Humana de UCLA, analizaron a 83 voluntarios con entre 49 y 84 años de edad: 25 eran pacientes con Alzheimer, 28 presentaban deterioro cognitivo leve y los 30 restantes eran controles sanos. Todos ellos fueron sometidos a una PET cerebral una vez que se les hubo inyectado el nuevo marcador químico, llamado FDDNP, la molécula que se vincula con las proteínas beta amiloide y tau.
Las imágenes de la tomografía mostraron que cuanto más avanzada estaba la enfermedad, mayor era la concentración de FDDNP en las zonas donde se acumulan típicamente los depósitos característicos del Alzheimer (regiones cerebrales temporal, parietal y frontal).
De hecho, las imágenes obtenidas indicaban la progresión de las lesiones cerebrales desde el deterioro cognitivo leve hasta el Alzheimer.
"El estudio sugiere que disponemos de una nueva herramienta de diagnóstico de las condiciones previas a la enfermedad de Alzheimer, quizá incluso antes de que los primeros síntomas hayan hecho su aparición", afirma el autor principal del trabajo.
En comparación con otras técnicas de imagen, la PET combinada con FDDNP diferenciaba mejor a los pacientes de la PET a la que se añade otro marcador químico, llamado FDG, con el que se mide la función metabólica de las células y que también se ha empleado en el diagnóstico del Alzheimer. Los investigadores también realizaron un seguimiento de doce sujetos incluidos en este trabajo dos años después, y comprobaron que aquéllos que evolucionaban peor tenían mayores aumentos de la vinculación del FDDNP con las proteínas, indicando cúmulos patológicos.

TenerifeEl Servicio Canario de la Salud (SCS) ha adjudicado mediante concurso el suministro e instalación de dos aparatos de Tomografía por Emisión de Positrones (PET) con destino al Hospital Universitario de Gran Canaria Dr. Negrín y el Complejo Hospitalario Nuestra Señora de Candelaria-Ofra. El valor de la inversión realizada es de 4 millones de euros. El PET (Tomografía por Emisión de Positrones) es una moderna tecnología que utiliza trazadores de vida ultra corta y se aplica fundamentalmente en Oncología y Cardiología.
